Moving from children to adults services

As you become a young adult, you will start thinking about what you want to do when you leave school and what support you may need to help you do this. We call this change transition. 

Until the age of 18, services for children and young people with health and social care needs are provided by children's social care services. From 18, support is provided by adult social services. 

Redbridge Transition Team 

The Transition Team will work closely with colleagues across children’s social care services to support you through transition from childhood to adulthood. 

The team provide advice, assessment, planning and support. They aim to help make the transition into adulthood for you as easy and smooth as it can be.  

Education, Health and Care Plan (EHCP) 

Your education, health and care plan (EHC plan) will be the starting point for planning your move into adulthood and you may receive some support until you are 25. The EHC plan will help you to continue with education or help you look for a job. 

We will talk to people who can help you think about what is important to you and what you would like to do in the future. 

These talks will help us make a transition plan for you. 

Charges 

When you turn 18 years old, you may have an assessment of need which is conducted by Redbridge Adult Social Services to check if you have eligible needs.  

We will carry out a financial assessment to determine whether the council is able to pay for some or all of your care and support services or whether you need to contribute towards these costs. This is different from children’s social care where the support given is free.
